Incident Type	Definition	Examples
Check Forgery	Your check has been counterfeited or altered by changing the amount of the check to something that you did not authorize or by changing the payee of the check to something/someone you did not authorize.	You mailed in a check to pay for a service. When your check cleared, you noticed that the amount was increased and the payee was different than what you made the check out for.
Fraudulent Use of Credit Card	Your credit/debit card has been compromised, which resulted in financial loss.	Your credit/debit card has been skimmed, cloned, or compromised resulting in unauthorized transactions.
Identity Theft	The taking and use of someone else's personal identification information to obtain credit, goods, or services.	Someone obtains a credit card or some other goods or service by use of your personal information (such as: name, Social Security number, date of birth, etc.).
Lost Property	Personal item that is missing with no indication it was stolen.	You lost your watch while walking downtown.
Online/Phone Scam	You were contacted online or via telephone and, as a result of this contact, you deposited a fraudulent check into your personal account or you purchased gift cards and provided the other party with the gift card numbers.	You purchase some type of gift card or money order and funds are delivered to another person resulting in a financial loss.
Property Damage (Non-Criminal)	When damage has been caused to your property as the result of a NON-criminal action.	Damage caused by natural events such as storms or wildfires. Also, damage unintentionally caused to property requiring a report number for insurance purposes.
Self Checkout Scam	Theft occurred at self checkout which resulted in financial loss.	Underpaying by not scanning all items, scanning cheaper item underneath more expensive item, cash back scam (buys cheap item, requests cash back, then tells customer service never got money).
Theft	When your property is taken without your permission, including theft of fuel from a motor vehicle. Lost property is not a theft.	An item was stolen from your yard, driveway, porch, or other common shared areas such as garages or laundry facilities of apartment complexes. Your fuel was siphoned from your vehicle's gas tank. A delivery company placed a package addressed to you on your front porch and now it is gone.
Theft from Vehicle	Stolen equipment or belongings from a vehicle.	Your jacket is stolen from the backseat of your vehicle, your hubcaps are stolen.
Vandalism	A deliberate act of damaging, defacing, or destroying property or a vehicle.	Mailbox maliciously damaged, broken windows, having eggs, shaving cream, or toilet paper thrown at house or property, keying a car's paint finish, etc.
